Every surgical wound should be evaluated and classified independently.

 

Wound classification should be documented by the perioperative RN in the patient record after each operative procedure.

 

The AORN Surgical Wound Classification Decision Tree is designed to enable perioperative nurses to accurately classify surgical wounds based on the CDC surgical wound classification system (AORN Tree). The decision tree asks a series of yes or no questions that enable perioperative nurses to accurately classify wounds based on the descriptions in the CDC classification system. The decision tree is found in the AORN Guideline for Prevention of Transmissible Infections.
